My name is Harshit Lohiya, 5 months back I left my job, yesterday I sent the Form 10 C & 19 to my company\'s HR Department for PF Withdrawal, I\'ve attached the Canceled Cheque, but unfortunately I forgot to attach the Bank Account Statement, Please tell me is this mandatory to attach the Bank Account statement along with the Canceled Cheque for PF withdrawal.

The correct details with regard to account number, name of the bank, branch address, MICR* code of the bank have to be filled correctly, as far as they are correct you need not worry regarding your account statement............
in any case the concerned department will cross check the affixed documents, in case there is any requirements you be informed to fulfill the same.....
---*(MICR is a 9 digit number printed on the cheque leaf, next to the cheque number)
